Kaleidoscope shortlisted for two HSJ Partnership Awards
Kaleidoscope has been shortlisted for two awards for its work with NHS England and Frimley Health and Care ICS to develop an innovative approach to managing heart failure.

The HSJ Partnership Awards recognise outstanding dedication to improving healthcare and effective collaboration with the NHS.
The work has been shortlisted in two categories: ‘Most Effective Contribution to Improving Care for Those with Long-term Conditions’ and ‘Best Consultancy Partnership with the NHS’.
The pioneering Managing Heart Failure @home (MHF @home) partnership developed an approach that combined remote monitoring, personalised care and integrated service delivery.
Kaleidoscope’s research and engagement expertise complemented NHS England’s clinical knowledge, while Frimley, an early adopter site, provided real-world testing of the approach.
Improved patient outcomes
Clare Allcock, part of the team at Kaleidoscope who worked on the project, said:
“We’re delighted to have been shortlisted for these awards in partnership with NHS England and Frimley. Our aim in all of our work is to help improve health and care and it’s a privilege to have contributed to a project that has clearly had a positive impact on patients and staff.”
The partnership has significantly improved patient quality of life, outcomes, resource allocation and service delivery. Its success has led to wider NHS adoption, with a blueprint supporting implementation in other healthcare settings, establishing it as a transformative model for long-term condition management.

Lizzie Cardis, Programme Manager, Delivery Unit (NHS @home – Long term conditions), NHS England, said:
“Being shortlisted for two awards is a fantastic achievement for all those involved in this work. It’s recognition not just for the successful way we worked together and with stakeholders, but also for the improved patient care the project has achieved.”
Eva Allende, Community Heart Failure Specialist Nurse at Frimley Health NHS Foundation Trust, said:
“At Frimley, the MHF @home project has transformed care for patients with long-term heart conditions. It significantly improved patients’ self-care and our ability to provide timely interventions. We’re delighted to see this reflected in being shortlisted for these awards”

Now in their eighth year, the HSJ Partnership Awards, have become the most recognised and respected mark of the strongest relationships between suppliers and the NHS.
The winners will be announced at the awards ceremony on 20 March 2025.
